Every patient is supported by a full team working together — not a single provider working alone. Each discipline brings its own expertise, and the team meets regularly to keep one coordinated plan of care centered on the patient and family.

Clinical Team

The medical backbone of every care plan

Physician / Medical Director

Oversees the medical plan of care and guides diagnosis and treatment decisions.

Responsibilities
  • Oversees medical care plan and diagnoses
  • Makes and approves treatment decisions
  • Coordinates medical referrals and clinical escalation

Nurse Practitioner

Provides clinical assessment and works alongside the physician to keep the plan current.

Responsibilities
  • Clinical assessment and diagnosis support
  • Develops and updates the care plan with the medical director
  • Orders medications, adjusts doses, monitors side effects within scope
  • Communicates clinical findings to the rest of the IDT

Nurse (RN / LPN / Case Nurse)

Delivers regular visits and hands-on symptom management for the patient.

Responsibilities
  • Monitors patient condition and vital signs
  • Administers treatments and medications
  • Tracks progress and communicates changes to the team

Pharmacist

Keeps every medication regimen safe, effective, and as simple as possible.

Responsibilities
  • Reviews medication regimen for safety and interactions
  • Recommends optimizations, including dose timing and deprescribing when appropriate

Coordination & Support

Keeping care connected and families steady

Care Coordinator / Case Manager

Keeps services, transitions, and paperwork moving smoothly behind the scenes.

Responsibilities
  • Schedules services and follow-ups
  • Manages care transitions — admissions, discharges, transfers
  • Ensures resources, authorizations, and continuity of care

Social Worker

Helps families navigate the practical and emotional side of the care journey.

Responsibilities
  • Addresses psychosocial needs — family support, coping, stress
  • Assists with benefits, housing, transportation, and counseling resources
  • Helps with discharge planning and community supports

Chaplain / Spiritual Counselor

Offers spiritual support tailored to each patient and family's beliefs.

Responsibilities
  • Assesses spiritual needs and provides comfort and support
  • Facilitates spiritual practices and rituals as requested
  • Supports patients and families, including at end of life

Bereavement Counselor

Supports families through grief, both before and after a loss.

Responsibilities
  • Provides grief counseling and education to families
  • Assesses for complicated grief and risk
  • Offers coping support and arranges follow-up or referrals as needed

Patient & Family / Caregiver

Often the most important members of the team — their voice sets the plan.

Responsibilities
  • Share goals, preferences, and concerns
  • Participate in education and home-care planning
  • Provide feedback on what's working and what isn't

Executive Director

Ensures the whole team has what it needs to deliver on the plan of care.

Responsibilities
  • Supports a culture of patient-centered, coordinated care
  • Ensures the IDT has the tools and authority to implement care plans effectively
  • Ensures the IDT operates in line with policies, regulations, and standards of care
  • Oversees resources and staffing so the right disciplines are available
  • Monitors quality outcomes — performance metrics, audits, compliance
